Title:Application for Issue and Renewal of Additional Ratings in Accordance with Part-FCL (Aerobatics, Towing, Mountain and Flight Test ratings).
Description:This form is used by professional and private pilots to apply for one of the following:
• Aerobatic rating
• Mountain rating
• Banner towing rating
• Sailplane towing rating
• Flight test rating

This application is now available as an online form.

Course Completion Certificate CAA5020 should be used with this application.

Applications for class, type or instrument ratings should be made using online form SRG3108.

Applications for night ratings should be made using online form SRG1126.
